Symmetric avascular necrosis of the shoulders and hips in sickle cell disease

A 19-year-old man with sickle cell disease had acute pain in multiple joints. Previous bone scans showed enhanced uptake at the end of his long bones and areas of heterogeneous uptake. Based on clinical evaluation, these episodes were thought to represent bony infarcts. Current scintiphotos show symmetric photon-deficient areas in the humeral and femoral heads consistent with avascular necrosis.
