Abstract
A 36-year-old man with keratoconus developed progressive visual field loss in both eyes. He had optic atrophy bilaterally. His original evaluation was normal with the exception of a positive FTA-Abs. Subsequent history revealed that the patient had a peculiar habit of proptosing his eyes. While he proptosed his eye the IOP was in excess of 70 mm Hg. Funduscopy with fluorescein angiography during such a maneuver demonstrated absence of arteriolar filling. Possible mechanisms producing the progressive visual loss are discussed.
