Cholesterol crystal pseudoascites: An unusual presentation of ovarian cyst

Abstract

Cholesterol crystals in body fluids may impart a characteristic shimmering 'gold paint' appearance to the fluid. We recently managed a patient with a massive ovarian cyst, in whom paracentesis revealed a gold paint-like cholesterol-containing fluid, initially misdiagnosed as ascites. Upper gastrointestinal series demonstrated the presence of a huge intraabdominal cyst and surgical extirpation of the cyst was performed. This report reviews the origin and importance of cholesterol crystal-containing body fluids and cautions that all abdominal fluid is not necessarily ascites.
