This is an observational cohort study over a 5 year period from October 2003 through July 2008 that has a recruitment goal of 540 children with mild to moderate kidney dysfunction with an estimated glomerular filtration rate from 30 to 90 ml/min/1.73m2 ranging in age from 1 through 16 years. 402/540 (75%) enrollment goal as of 1.16/07 has been meet with a retention of 95.3% at 6 month follow up visit. The goals of the study are to define risk factors for chronic kidney disease (CKD) progression and to define the effects of CKD progression on: neurocognitive developmental function, prevalence of cardiovascular disease risk factors, and growth failure. The scientific domains and key measures involve questionnaires and forms, iohexol-based GFR, cystatin C and serum creatinine, echo and ambulatory blood pressure measurement, cognitive development and behavioral assessment, tanner staging and parathyroid hormone measurement, and stored biological samples and genetic studies.
